MEDICAL DIRECTOR

Responsibilities:  Oversees all clinical activities of the Boriken Neighborhood Health Center and other East Harlem Council for Human Services programs. Plans, organizes, supervises and evaluates health care services, including primary care, diagnostic, treatment, therapeutic, support, health education and other health-related services. Ensures the coordination of referrals to other agencies and providers for services not available onsite. Responsible for oversight of performance improvement and clinical risk management activities, development of new and evaluation of existing clinical programs, training of health professionals including medical, nursing, dental and other students and residents/fellows, maintenance of an appropriate complement of competent, trained health care staff and up-to-date clinical and operational policies and procedures.  Maintain limited patient care schedule.

 

Qualifications: Board certified or eligible physician (Internist, Pediatrician or Family Medicine Physician) with experience in public health. Medical professional with excellent organizational, management, written & oral communication and interpersonal skills. At least 10 years of experience providing primary care and five years of administering services. Understanding of requirements of Patient Centered Medical Home (NCQA), Meaningful Use, value-based payment methodologies proposed for NY State and DSRIP. Master in Public Health preferred. Bilingual (Spanish) preferred.  Commitment to provide the highest quality of patient-centered, culturally competent care to diverse patient populations.

 

NURSING SUPERVISOR

Responsibilities: Oversees all of the nursing functions of the Boriken Neighborhood Health Center.

Qualifications: Current New York State Registered Nurse License. CPR certification. Infection Control Certification. Excellent written, verbal and interpersonal skills. Knowledge of general nursing theory, practice, methods and techniques involved in the performance of general services. Bilingual Spanish preferred.

 

CLINICAL QUALITY ANALYST

Responsibilities:  The Clinical Quality Analyst serves as the bridge, between the informatics, clinical and administrative departments, to promote quality care. The Clinical Quality Analyst uses data generated from the electronic medical record and other sources to implement and continuously maintain systems and processes to improve the quality of care rendered to the patients of the East Harlem Council for Human Services, Inc. Boriken Health Center and its School Health Program (Boriken).

 

Qualifications: Registered Nurse, Physician Assistant or Nurse Practitioner with a minimum of 1 year of Quality Improvement experience in addition to 4 years of clinical experience. Proficiency with eClinicalWorks. Super-user preferred. Experience in quality improvement or program coordination. Excellent data analysis skills, basic understanding of statistics, program evaluation methods, and program planning tools/methods. Experience in a community-based health center or FQHC setting preferred. Ability to take the initiative and utilize innovative techniques. Experience in producing clinical report documents from source databases. Ability to work with a diverse group of clinical, administrative and executive staff members. Excellent organizational and interpersonal skills. Excellent communications skills, including writing and presentation skills. Ability to work in a dynamically changing environment with competing priorities. Obtain and/or maintain current proficiency the EMR through continued education/learning.

DENTAL ASSISTANT

Responsibilities: Assist dentist chair-side during examination and treatment of patients. Prepare operatory instruments and supplies for the scheduled treatment procedure. Seat and prepare patients for treatment. Prepare restorative materials and cements as required and pass instruments to the dentist. Take, develop, mount and file radiographs. Dismiss patients and disinfect the operatory. Provide post-operative instructions as required. Maintain an inventory of supplies. Clean dental operatory as needed. Clean and maintain dental equipment, including hand pieces on a regular basis. Monitor laboratory cases by logging cases leaving and returning to the health center. Pour stone or plaster models and assist the dentist with laboratory work as needed. Sterilize instruments by the appropriate method and return them to proper drawers or trays. Clean and maintain the dental laboratory, vacuum machine, x-ray equipment and autoclaves. Monitor sterilization equipment with biological indicators. Record notes and data in dental chart. Obtain patient medical history and blood pressure; record information in dental chart. Assist in the reception area as needed.

 

Qualifications: Bilingual Spanish preferred. High School Diploma or G.E.D. Certification from a Dental Assistant Program.

 

OUTREACH WORKER

Responsibilities:  Provide educational outreach, preventive and referral services to all patients/clients of the Council. 

 

Qualifications: High School Diploma or GED, NYS Driver’s License, 2 years of experience in health education and outreach. Excellent interpersonal and communication skills. Reliable. Flexible schedule. Quick Learner. Bilingual Spanish/English required.
